Request a current, written estimate from the hospital or clinic after it explains what information it needs to review. The amount and scope may depend on the department's assessment, tests, treatment plan and service arrangements. Confirm what is included, what may change and how payment or insurance is handled.

Request an estimate with a defined scope
- Name the consultation, test or service being estimated and the records reviewed.
- Ask whether clinician assessment, imaging, laboratory tests, medicines, interpretation and follow-up are included.
- Ask how long the estimate is valid and which clinical changes could affect it.
- Confirm deposits, payment timing, accepted payment methods, currency and refund rules.
Check insurance separately
Ask the insurer to confirm coverage and pre-authorization for the named provider and service. Ask the hospital whether it bills the insurer directly or requires the patient to pay and claim reimbursement. A policy card or network listing alone is not a coverage decision.
Keep coordinator fees separate
If a separate access or coordination service is offered, request its own written scope and fee schedule. Do not combine it with hospital consultation, diagnostics or treatment charges.
